Fees for Services

Our office fee includes the physical examination, a Tuberculin skin test, required blood tests and completion of your application.

If you require immunizations, you may receive them for an additional fee.

If you require a chest x-ray you will be referred to a nearby diagnostic facility.

We accept cash or major credit cards for payment.

Your private medical insurance will not pay for the cost of your examination or immunizations.
